Central Florida Council BSA Accident/Incident Report Form (.pdf file - Adobe Reader):  Units and districts to use reporting major or recurring accidents/incidents involving increased safety problems that increase our risk.   After the report is sent to the Council Risk Management Committee, it will be reviewed at the next quarterly Risk Management Committee meeting.  Then the Risk Management Committee will reply to the unit on the safety issue.  The committee will also look at the need to forward this accident/incident report on to the National Risk Management Committee.
